It is with mixed feelings that I am informing you about the upcoming changes to my practice and how it may affect you. From August 15th, 2025, I will transfer all my patients to Dr. Uche Nwokpor.

Dr. Nwokpor works at Ebenezar Clinic, Red Deer, AB. He is an excellent and conscientious Family Physician.

I have truly loved and cherished being allowed into your lives and families to serve your healthcare needs, I do not take this for granted. I would use this medium to express my appreciation for the trust and therapeutic relationship that you shared with me.

I understand that this news may come as a surprise and could raise concerns about your ongoing medical care. Please be assured that I am committed to making this transition as smooth as possible for you. As I had stated earlier, I have arranged to transfer your care to Dr. Uche Nwokpor.

Please feel free to book a Meet and Greet with Dr. Nwokpor at your convenience.

You are at liberty to move your care to any other physician of your choice. We have two other physicians taking new patients in the clinic that you may wish to consider joining: Dr. Priye Caleb and Dr. Eghosa Egharevba. Alternatively, you can search for physicians

taking new patients a on CPSA website: www.cpsa.ca “Find a Physician.”, You can narrow down your search for physicians in your area with criteria such as gender and languages spoken. Another option may be to contact Red Deer Primary Care network- 403-343-9100. They can provide you with the names of local physicians who are accepting new patients in Red Deer.

I will continue to see patients at the clinic for management of medical conditions, refill of prescriptions and any other health-related needs during this transition period.

Please reschedule appointments pre-booked for August 2025 and beyond. , please call the clinic to reschedule.

Your health records will remain at Ebenezar Medical and Walk-in Clinic and can be accessed by Dr. Uche Nwokpor or any physician that you join in the clinic If you prefer to change clinics, your medical records will be transferred to your new physician.

I have truly loved and cherished being allowed into your lives and families and I do not take it for granted. I thank you immensely for entrusting me with your health for these past years. This is not a decision that I came to easily, but one that I must make for family reasons.

My colleagues Dr. Priye Caleb and Dr. Eghosa Egharevba both practicing at the Ebenezer Medical and Walk-in Clinic, Red Deer have agreed to take on my patients.
